Some of my information is wrong. How do I correct this?
If you notice an error in your personal information, such as address and phone number, please update your account information by using the links displayed on the left side of the page. The Account Info link will allow you to update general information as well as insurance. You can also update your personal and family history by using the history link.

How do I communicate my medical concerns?
Superior Access should not be used to communicate emergency medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL YOUR PHYSICIAN’S OFFICE OR CALL 911.
Your test results are available by clicking on the Chart link on the home page of Superior Access. Once you click on the Chart link scroll down to the checkbox labeled Results(Labs, Xrays, Orders), check the box and then click on the View Chart as PDF button. Please note that not all results are available. You can send a message through the Superior Access Portal or call the office to request your results.
How do I get the complete set of my medical records?
You can request a copy of your medical records from your physician’s office, or you can download your medical record from the Chart link of the Superior Access Portal by clicking on the Select All checkbox and then clicking on the Download Zip File button.
I don’t have an email address, how do I get one?
There are many free email services available to use. Any email service is compatible with Superior Access. If you are looking to create a free email account, some of the more established email providers are:
How can I update my email address in the Superior Access Portal?
To update your email information simply click on the Edit Profile link at the top of the Superior Access home page.
What information can I see in the Superior Access Portal?
You can view or print information from your medical record such as laboratory results, family and social history, medications, past visits, allergies, current health conditions, and medical summary. You may also update your demographic information and personal information. Is there a fee to use the Superior Access Portal?
Superior Access is a FREE service offered to all of our patients.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. Superior Access uses the latest encryption technology. All Superior Access messaging is done while you are securely logged on to our portal.
What if I need more information than is available through Superior Access?
If you need additional information than what is available through the Superior Access Portal, please contact your doctor’s office or send a message through the portal.
You can request an activation code online by visiting requesting an activation code once the form has been received and processed, you will receive an activation letter in the mail detailing next steps. Or you can pick up the letter at your physician’s office. Processing normally takes a week for the letter to be mailed to the address specified. You can also request an activation letter on your next visit.
