Southeastern Med is pleased to offer good faith estimates for the price of hospital services. Estimates do not include fees for professional services provided by a physician, surgeon, radiologist, pathologist, anesthesiologist or other independent practitioner. You may receive bills from multiple physicians/practitioners who participate in your care.
You can request an estimate of hospital prices using the form below. When requesting a price for diagnostic tests or surgical procedures, it is helpful to know the “CPT” or billing code(s). Your physician’s office may have these codes, or if not available please provide a detailed description of the service to be performed. This will help us provide an accurate estimate. Please understand that if the description of the service is not accurate and complete, the price estimate quoted will not be accurate. In order to give a more accurate estimate, we may call your physician to confirm your procedure and diagnosis.
All online requests will be answered within one business day. We work very hard to ensure our estimates are as accurate as possible, but price estimates are provided without information as to all actual services that may be needed and provided for your care and therefore may not reflect actual charges on the final bill. Actual charges will vary based on the patient’s medical condition, supplies used, complications, length of stay, and/or additional treatment recommended by your physician.
Your insurance benefits – including your responsibility to pay deductibles, co-insurance and copayments – may also affect your final out-of-pocket liability for healthcare services provided by Southeastern Med. Uninsured or underinsured patients should consult with our Patient Financial Services staff to determine whether they qualify for discounts.
